Lack of thought can be dangerous

Alas, poor design seems more prevalent than good design. In general, design failures are caused by lack of thought, not by malicious intent.

Automobile radios that cannot be operated safely while driving. Flap and landing gear levers on private private aircraft that look the same and are located next to each other. Ford has a foot brake release and hood release that look the same, and are positioned next to each other.

Yup. You meant to raise the flaps and instead raised the landing gear? A very expensive accident. Meant to release the automobile’s brakes and the hood pops open? Lack of thought can be dangerous.
